Agent guidanceHow can agents determine whether a group is a good fit before quoting?
Review the group’s employee count, expected enrollment, budget, locations, key doctors and medications, and renewal date. Each major medical product requires at least five enrolled employees, and the offering also requires CHAMP participation. Part-time employees and 1099 contractors are not eligible for these major medical options. Confirm eligibility, provider access, and any costs or obligations associated with leaving the current plan.
AvailabilityAre MVP Select PPO™ and Premier PPO™ available nationwide?
Yes. MVP Select PPO™ and Premier PPO™ are available in all 50 states. Check the provider network for your selected plan to confirm access to doctors and hospitals in your area.
EligibilityHow many employees must enroll in an AffordaCare major medical plan?
At least five employees must enroll. If you offer both MVP Select PPO™ and Premier PPO™, each product must have at least five enrolled employees.
EligibilityWhat CHAMP participation is required with AffordaCare major medical coverage?
At least 25% of employees enrolled in the major medical coverage must also enroll in CHAMP. The CHAMP group must have at least five enrolled employees overall. Both requirements apply.
QuotingWhat information do I need for a major medical quote?
No documents are required for an initial quote under the standard fixed-rate offering. For a meaningful comparison with your current coverage, provide your latest premium or renewal invoice and a summary of benefits. A fully underwritten quote has separate documentation requirements.
RatesWhat determines the premiums for AffordaCare major medical plans?
Premiums for the standard guaranteed-issue offerings are not based on an employee’s age, gender, or health history. The selected plan and benefit design determine the applicable rate. An alternative fully underwritten quote, when available, follows a separate review process.
EnrollmentWhat is required to start AffordaCare major medical coverage?
The employer must submit a completed enrollment census listing the employees and dependents enrolling, their selected plan, and required personal information. The employer must also complete the intake form and sign the plan documents. These materials are used to set up the group and issue ID cards. Work with your agent to confirm the submission deadline and effective date, and use a secure upload link for sensitive information.
Provider accessHow do I check whether a doctor or hospital is in my major medical network?
Check the network listed for your selected plan or on your ID card, then use the matching provider directory: Cigna PPO: https://hcpdirectory.cigna.com/web/public/consumer/directory; PHCS PPO: https://providersearch.multiplan.com/; First Health PPO: https://www.firsthealthlbp.com/. Confirm participation for your specific plan before scheduling care.
OverviewWhat is the CHAMP Plan, and what benefits does it include?
The CHAMP Plan™ is an employer-sponsored program that supplements major medical insurance. It combines minimum essential coverage with a health population management program. Benefits include covered primary and urgent care visits at participating providers with $0 copays, preventive care, virtual care, personal health assistance, and prescription support. Eligible household members can access specified virtual-care services after registration. Benefits depend on the plan version, so review your plan materials for coverage details.
Agent guidanceHow should agents compare AffordaCare major medical plans with other level-funded options?
Compare plans with similar deductibles, copays, provider networks, and prescription benefits. Then review claims administration, stop-loss protection, any return of unused funding, renewal pricing, and costs when the employer leaves the plan. Include CHAMP participation and costs in the total comparison. AffordaCare’s comparison tool is available at https://compare.affordacareinsurance.com/.
